PATHWAY 1 (Self-Referrals & Organisation Referrals)

Financial Hardship (supporting evidence required):

• Receiving either: universal credit, pension credit or carer's allowance.

AND

High Rehab Urgency Criteria (one or more)

AND

Awaiting NHS physiotherapy input or recently discharged

PATHWAY 2 (Organisation Referrals Only)

Special Circumstances:

• Homeless individuals
• Asylum seekers and refugees

AND

High Rehab Urgency Criteria (one or more)

AND

Awaiting NHS physiotherapy input or recently discharged

Please note that the criteria must be met at the time of referral to ensure that individuals are eligible for this service.

Our initiative prioritises patients with high urgency conditions to ensure we support those facing the greatest medical, mobility and financial challenges.

Severe Mobility Loss (last 4 weeks)

• Newly housebound and needing significant assistance with mobility and daily activities.

Recent Fall (last 4 weeks)

• A recent fall that led to hospital admission and injury (i.e., fracture, head injury).

Stroke or Major Neurodegenerative Decline (last 4-6 weeks)

• Stroke in the last 6 weeks leading to severe mobility loss.
• Recent and sudden worsening of Parkinson's, MS and other neurodegenerative conditions causing severe mobility loss (last 4 weeks).

Post-Operative Trauma (last 4 weeks)

• Post-surgery following severe trauma (e.g., fractures, dislocations, major soft tissue injuries), where early rehabilitation is needed to regain function and mobility.

Please attach a hospital discharge summary onto the referral form if applicable.

If selected, you’ll be contacted with your appointment details—please confirm promptly. 

Availability depends on our capacity; if you can't attend, the slot may be offered to another patient, and you'll need to re-refer.

If you haven't heard back from us within a month, you may submit a new referral if you still meet our criteria.
