Case Managed Physio

Our physiotherapy services within managed care programs are designed to support recovery through structured, goal-oriented rehabilitation—not medicolegal evaluation. We work collaboratively with case managers to provide consistent, high-quality care focused on restoring function and improving patient outcomes.

All assessments, progress updates and treatment plans are strictly clinical in nature. We do not provide medicolegal reports or expert opinions for legal proceedings. Our role is to assist with rehabilitation planning and return-to-work or daily activity goals under a coordinated care model.

General Functional Assessment

Our General Functional Assessment (GFA) Service delivers a meticulous and evidence-based evaluation of your physical capacity, specifically designed to provide detailed, objective clinical evidence of your function.

This service is focused on assessment and detailed report formulation, documenting your mobility, functional limitations, and ability to perform necessary daily activities using standardised outcome measures, not on immediate rehabilitation planning. The resulting professional report establishes a clear, measurable picture of your current functional status for use in various contexts.

DISCLAIMER: The assessment and report provide detailed and objective clinical evidence of your mobility and function—not medicolegal opinions. GoMove Physio and its team do not accept Blue Badge, PIP or ESA applications. Eligibility is determined solely by the Local Council/Issuing Authority/DWP. Submission of this supportive evidence report does not guarantee approval.

Reporting & Fees

All reports are strictly clinical and descriptive, designed to support administrative, case coordination, and application processes—not medicolegal opinions.

Administrative Note: All reports in Tiers 2 and 3 incur a separate administration/writing fee listed below. This fee is for professional documentation time and is in addition to the clinical assessment fee.

Tier 1 (free):

Discharge summary to GP and patient (upon request only).
Referral letter to another provider.
Mandatory safeguarding referral and reports.
Statutory Fit Note >7 Days (if deemed appropriate).

Tier 2 (£30):

Brief progress report for case managers (if requested).
Statutory Fit Note <7 Days (if requested).

Tier 3a - Detailed Case Managed Report (£60):

Detailed initial assessment and final reports for case managers.

Tier 3b - Detailed Functional Assessment Report (fee included in the GFA service or £70 for the report only):

Detailed functional supportive report for applications (Blue Badge, PIP, ESA), work abilities & overall mobility.
